Assessment and referral requests
The public forms send contact, organization, service, location, timing, and brief non-clinical messages to SNS for business inquiry and intake coordination. Submissions may be recorded in an SNS Google Sheet used only for lead and scheduling follow-up. SNS does not ask for clinical records through the public website.
Do not send clinical details
Do not submit names of prospective clients or residents, diagnoses, medications, dates of birth, Social Security numbers, insurance identifiers, or clinical records through the public website or ordinary email. SNS will provide separate instructions when records are needed.
Authorized communication
Before discussing or releasing clinical information, SNS may need to confirm the identity and authority of the requesting party or the individual's authorized representative.
