When Must a Washington AFH Update a Negotiated Care Plan?

What the rule requires

• After an assessment identifies a significant change in the resident’s physical or mental condition
• When the plan, or part of the plan, no longer addresses the resident’s needs and preferences
• At the resident’s or resident representative’s request
• At least every 12 months

What this means for an AFH

The home should not rely only on an annual calendar reminder. A significant change, an outdated instruction, or a resident request can require earlier review and revision.
